Residential care providersClinical pharmacist support for residential care providers

We help group homes, adult residential facilities, ICF/DD homes, RCFE and assisted living keep medications safe, documented and ready for licensing visits, working with the pharmacy you already use.

Pharmacist oversight depends on your license type.

ICF/DD homes

A pharmacist must review each person's drug regimen at least quarterly. 42 CFR 483.460(j)

Regional center vendors

Any medication error must be reported to the regional center within 24 hours. 17 CCR § 54327

ARF and RCFE

No equivalent pharmacist requirement, even though residents often take as many medications. That's the gap we fill.

Safer medication practice in every home

Direct support staff give most medications, turnover is constant, and a single discharge can change a whole regimen. We work with your staff in the home as well as reviewing records.

Settings we support

  • Adult residential facilities (ARF) and group homes
  • ICF/DD, ICF/DD-H and ICF/DD-N homes
  • Residential care for the elderly (RCFE)
  • Assisted living communities

Questions from residential providers

Can you help us prepare for a licensing visit?

Yes. Mock reviews check medication storage, administration and records against your requirements, and we help with any plan of correction.

Do we have to change pharmacies?

No. Clarity Med is independent and works with the pharmacy your home already uses.
