If you or someone you care for takes several medications, sees more than one prescriber or was recently in the hospital, a pharmacist can check that everything works safely together. You get answers in plain language, and your prescribers get our recommendations.
When a review helps
Each prescriber sees part of the picture. A pharmacist looks at all of it: prescriptions from every doctor, over-the-counter products and supplements, and how you're actually doing.
Good reasons to ask
- Five or more medications, or several prescribers
- A recent hospital stay or ER visit
- New falls, confusion, sleepiness or behavior changes
- Blood thinners, diabetes, seizure or psychotropic medications
- Caring for an aging parent or an adult with a developmental disability
- Medications that seem like too many, or aren't helping

Cost and coverage
Fees depend on how many medications are involved and how much follow-up you need. You'll get a quote before anything starts.
If you have Medicare drug coverage, your plan may offer a free medication review if you qualify. Call the number on your plan card to ask. We'll tell you if that's the better first step.
We don't prescribe or change medications. Your prescriber makes every decision, so please don't stop a medication before talking to them. In an emergency, call 911.
Questions from individuals and families
Do I need a doctor's referral?
No. You can contact us directly. With your consent, we send our recommendations to your prescribers, and they decide on any change.
Can you review medications for a family member?
Yes, with their permission, or yours if you're their authorized representative, such as a conservator or a health care agent. If they live in a care home, we can work with the home and their prescribers too.
Can I use an AI tool instead?
AI tools can explain general information, but they can't see your full medication record or lab results, can't contact your prescriber, and get drug questions wrong often enough to matter. More on using AI for medication questions.
Do you work outside California?
Not yet. Our pharmacist is licensed in California, so we review medications only for people who live in California.
