This website describes our services and shares general information about medication safety. It isn't medical advice, and it isn't a substitute for the judgment of the treating prescriber or your clinical team. Using the site, reading a checklist or sending us a message doesn't create a pharmacist–patient relationship or a consulting engagement; those begin only under a written agreement.

Research and results

We summarize published research and regulations to explain why medication review matters. Research findings describe what happened in those studies and aren't a guarantee of results for any person or organization. Regulatory summaries are for orientation only; always check the current text of a law or regulation, and seek legal advice where needed.

Using our checklists and guides

You may print, copy and share our checklists and guides for use within your own organization, homes and programs, and adapt them to your policies, as long as you don't sell them or present them as published by someone else. If you change a checklist, please remove our name or note that it has been adapted. They're general tools: they support, and don't replace, your policies, your licensing requirements, each person's care plan and the prescriber's instructions.

Our content and brand

Other than as allowed above, the text, design, logo and other content on this site belong to Clarity Med and may not be copied for commercial use without our written permission. "Clarity Med" and the capsule logo are our brand marks.

Acceptable use

Please don't use the site or its forms to send spam, malicious code or anyone else's health information, or to interfere with the site's operation or security.

We link to research, regulations and other resources we think are useful. We don't control those websites and aren't responsible for their content or privacy practices.

We work to keep this site accurate and current, but it's provided "as is", without warranties of any kind, to the fullest extent the law allows. To the fullest extent the law allows, Clarity Med isn't liable for any loss arising from use of this website or reliance on its general information.
